Clinics and Services
6 to 8 Week Mum and Baby Check
About six weeks after you give birth, your baby should have a check-up with your GP and you should have a postnatal check-up appointment.
Your baby's 6 week baby check is a chance for your GP to give your baby a check-up to make sure they’re well. Your postnatal check-up is to see how you’re doing after the birth and how you’re feeling as a new mum.
ADHD and Autism Referrals for Adults
If you believe you require a referral for a diagnosis of ADHD or Autism, we can help you with this.
The process has become an increasingly complicated area to navigate with more and more options available. To help you consider the best way forward for you, we have put together the following information which outlines your options, and our responsibilities with respect to each.
We hope this information helps you decide how to proceed. If you would like to proceed with a referral, please complete the applicable forms for your pathway and send them to us at the Surgery. Your completed forms will be reviewed by a doctor, and we will contact you to let you know the next steps.
1. NHS Referral
- Via Community Mental Health Team
- Comprehensive service providing assessment, diagnosis and treatment including prescriptions and access to local support services
- We can consider taking on shared care (ongoing prescriptions and monitoring in general practice with specialist oversight) using Devon’s formally agreed share care guidelines
- For consideration by this service, please complete, and return to the surgery, either the ADHD or Autism referral form
2. NHS Right to Choose Referral
- Private providers that have been commissioned to provide assessment, diagnosis and treatment paid for by the NHS
- Often remote virtual care
- We can consider taking on shared care (ongoing prescriptions and monitoring in general practice with specialist oversight), only if they agree to use Devon’s formally agreed share care guidelines.
- The following website lists the Right to Choose providers who are currently contracted with Devon ICB, and agree to using Devon Shared Care Agreements.
- Please have a look at the above website for appropriate providers (bearing in mind that some only assess Autism, and some only adults or children) and follow the web address to find the referral forms for each provider.
3. Private Services
- We cannot share care with private providers. This means that you will be responsible for ongoing costs associated with any treatments and monitoring required (e.g. blood tests, ECGs, blood pressure). Your private provider will be responsible for providing this and any prescriptions.
- You can still be referred to the NHS service at any time, but they will not take over your care until you have been seen in their clinic.
Asthma and COPD Clinics
Run by a specialst respiratory nurse who is trained in the diagnosis and management of respiratory disease. Annual check ups are advised for all patients with asthma and COPD for monitoring, advice, inhaler use and any other query can be discussed. Appointments can be made through reception.
Breast Screening
This service is offered to women aged between 50 and 70 years. You will receive your first screening invitation by letter after your 50th birthday and prior to your 53rd birthday. After this you will receive an appointment every 3 years until you are 70. After the age of 70 you can refer yourself directly on 01392 262600.
Cervical Smears
It is national policy that all women between the ages of 25 to 65 years who have been sexually active at any time are advised to have a smear test. These are important as they can detect early signs of serious disease which can then be easily treated.
A smear appointment may be booked online or via our reception team.
You will be informed by letter from the local health authority when your next test is due and they will write to you with your result.
Child Vaccinations
One of the most important things that a parent can do for their child is to make sure that they have all their routine childhood vaccinations. It's the most effective way of keeping them protected against infectious diseases.
Ideally, children should have their jabs at the right age to protect them as early as possible and minimise the risk of infection.

Diabetic Clinic
Run by the diabetic nurse who is experienced and specially trained. The aim of this clinic is to achieve good control of diabetes and prevent complications by providing regular checkups and education. Appointments can be made through reception.
Family Planning
Advice is available from either a doctor or a practice nurse on all aspects of family planning including oral contraception, coil fitting and emergency contraception.
Flu Vaccination
We offer seasonal flue vaccinations. Keep an eye on our news page for any upcoming clinics

Joint Injections
We offer joint injections in a dedicated once monthly clinic. Patients will need to consult with a clinician who will identify where a joint injection may be appropriate and patients will be added to a waiting list and contacted by the practice to be booked into a joint injection clinic. The waiting list is between 8 and 12 weeks.
Joint injections are not offered by all GP practices. This is an enhanced service we have chosen to provide, beyong the standard GP services.
Additionally, you have the option of being referred to the hospital for treatment or seeking a private provider.
Minor Surgery
Dr Hallmark provides a 6 weekly minor surgery clinic with support from a Health Care Assistant.
Patients will need to consult with a clinician who will identify where minor surgery may be appropriate and patients will be added to a waiting list and contacted by the practice to be booked into a minor surgery clinic. The waiting list is 12 - 16 weeks.
Minor surgery is an enhanced service we have chosen to provide, beyond the standard GP services.
NHS Health Checks
The NHS Health Check is a health check-up for adults in England aged 40-74. It's designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk. Patients who are not already diagnosed with one of the above conditions will be invited, once every five years, to make an appointment with one of our nursing team.
Pneumonia Vaccinations
All adults aged 65 years and over are recommended to have a pneumonia vaccination. One injection protects for life. Appointments can be made through reception.
Smoking Cessation
Run by a health care assistant who is fully trained to offer advice to patients who want to stop smoking and they are offered ongoing support in this. Appointments can be made through reception.
Well Person Checks
Adults may book in for a general health screen. New patients will be offered an appointment soon after they register.
